ACTS Senior Services recognizes there is no place like home. We designed Wellness Services to help those who have recently been discharged from the hospital, skilled care center, or have had a change in health status while still at home. From a slight fever with a cold or flu, to a broken arm, or an acute arthritic episode, ACTS Senior Services offers Wellness Services in the comfort of your own home.

During Wellness visits, our staff member will assist you with activities of daily living, including personal hygiene such as bathing, dressing and grooming. Additional help could be arranged for chores you might not be able to do so easily any longer, such as changing linen or taking out the trash.

Wellness visits are generally scheduled by referrals from a nurse or social worker. A supervisory nurse from ACTS Senior Services will then contact you to schedule an appointment to assess your needs and discuss a tailored service plan.
